Health Yeppoon

patient information

Everything you need to know for your visit

A general practice with a difference

Results are only given to patients during a medical consultation with their doctor. Your doctor will advise you to make a further appointment at the time of the consultation when the particular tests are ordered.

Bookings

Bookings can be made in person, over the phone, or online. If you require a more urgent appointment, call the practice on (07) 4939 3041 and let our team know it is urgent. For an emergency please call 000.

Patients that require assistance with communication are asked to please advise reception at the time of booking the appointment. Interpreter services are available if needed.

Patient Forms

New patients of our practice are required to complete a new patient form.

If you would like to complete the new patient registration form or transfer of records form prior to your appointment, please click below. You may bring with you, or email prior.

Fees and Billing

Health Yeppoon is a mixed billing practice. Medicare rebates are available to those with a valid Medicare card. We offer Medicare Easyclaim, which allows us to immediately refund your rebate to your debit card (not available for credit-only cards).

Some items are bulk-billed to Medicare, such as health assessments and chronic disease management plans. Consultation fees depend on the length and complexity of the service provided.

We have both government funded and privately funded vaccines available, depending on your eligibility. This can be discussed with the doctor.

We do not accept cash or cheque payments.

Phone Calls and Emails

Please note that our email and social media accounts are not constantly monitored. If you wish to make an appointment, please phone our reception on (07) 4939 3041.

If you consent for us to email you information, you will be required to confirm your email address first (for security reasons).

If you wish to speak to your doctor, please make an appointment.

Results

Results are only given to patients during a medical consultation with their doctor. Reception staff do not have access to your results. To streamline this process and to ensure that you have appropriate follow up, it is best for you to make an appointment when the tests are ordered.

We may contact you via phone call or text message.

Reminders

To ensure that you receive the best preventative health care, we use a clinical reminder system. This includes reminders for immunisations and chronic disease management. We may contact you via phone call or text message.

After Hours

For medical emergencies please call 000 or attend Capricorn Coast Hospital. For after hour care, phone (07) 4939 3041 and follow the prompts. Please note, after hours consultations incur a charge. Alternatively, you can call 13 HEALTH (13 43 25 84) for confidential phone advice.
